what it claims

VO₂ max is the ultimate indicator (the "Holy Grail") for predicting and optimizing longevity and long-term health.

The concept of VO₂ max as a health marker is widely supported by scientific research. A major meta-analysis published in the British Journal of Sports Medicine (and confirmed by other large-scale observational studies) shows a very strong correlation between a high VO₂ max and a significant reduction in all-cause mortality. It is important to add nuance, however: while VO₂ max is an excellent indicator of cardiorespiratory capacity, the term "Holy Grail" is a marketing simplification. Longevity also depends on other essential pillars such as muscular strength, sleep quality, nutrition, and overall metabolic health. Peter Attia never suggests that this indicator is sufficient on its own, but he insists on the fact that it is often overlooked compared to other metrics. Science therefore validates the capital importance of this measure, while reminding us that it is part of a complex ecosystem of health factors.

What the graded evidence says

Evidence
B

Linked to, not proven to cause.

Effect
all-cause mortality by cardiorespiratory fitness (treadmill METs) — elite vs low fitness adjusted HR 0.20 (95% CI 0.16-0.24); mortality inversely proportional to fitness with no upper limit of benefit; n=122,007
Measured on
adults referred for exercise treadmill testing (single large health system)

What’s uncertain

The fitness-mortality link is observational and drawn from patients referred for treadmill testing, so it shows strong association, not proof of cause. Dramatic comparisons (e.g. 'more dangerous than smoking') are rhetorical, not precise.
